If you sign up for email updates, claim a listing, or request a correction, we collect what you type into the form (such as name, email, agency and message) and use it only for that purpose. Email updates include an unsubscribe link. Please never send patient or health information.

Our hosting provider processes standard technical data (such as IP address and browser type) to deliver and secure the site. We'll update this page before adding any analytics.

Agency data

Agency information on this site comes from public government sources (Medicare/CMS and Texas HHSC) and describes businesses, not patients.

You can ask us to delete information you submitted through a form by using the contact form.
